Are you clicking the "Add the file" green button after you choose the file?

The whole BBcode thing seems to be broken on my forum account.

Sending code also doesn't work at all.

Check your User Control Panel. I think you have BBCode turned off.

That's not a regex. Use the "String Match" trigger.
