Indigo definitely sent the command to perform the unlock, so then the question is what told Indigo to do that. It could be several things: remote access via Indigo Touch or web browser (devices page or a control page), the Indigo Mac UI, a script, a plugin, or an Indigo Trigger or Schedule with the checkboxHide Executes in Event Log enabled.
Presuming you have a strong password I doubt it was something nefarious if it came from Indigo Touch or the Web UI. Do you use Indigo Touch or the Web UI? If so, was it being used around the time it was unlocked (maybe an accidental touch on a Control Page control?)? Do you have any scripts that control your locks? Or Trigger or Schedules?
We have on our feature request / ToDo list to provide more detailed logging on the origination of sent commands like this, which will make troubleshooting this easier (in the future).
We have remote access, but only me and the wife access that.
A control page that is loaded up on an iPad out of reach of the kids (but doesn’t have controls to unlock doors on it).
No triggers or schedules to lock or unlock any doors.
It’s integrated with HomeKit via the plugin so possibly could have received a command from there?
