Insteon Wall Switch Stopped Controlling Load

Have an odd switch behavior. I have an Insteon wall switch that seems to function/respond normally, from the switch and remotely but doesn’t turn the light on/off.
Curious if anyone has seen a switch fail in this way?

I have has a switch act like that once, and then I killed another without cutting the power and shorting the output

I moved said switches to a slave in a 3 way switch configuration and they have been working in this configuration for 10+ years.

Thanks. Guess I’ll just replace it. It’s in a 3 way setup as well.
