ZW4006 Smart Motion Switch 26931 won't sync under 2021.2

Back in November (under indigo 2021.1) I installed a GE zwave smart motion switch (26931). It included and synced just fine and works great. I've since upgraded to Indigo 2021.2 and today tried to install another ZW4006. It included smoothly but won't sync:

I closed my server, and rebooted under 2021.1 and it sync's just fine. I also tried to re-upgrade to 2021.2 after the sync but to no avail.

Any ideas?
