Jay,

So to summarize the steps I learned I needed to go through to add/replace/change/modify Control Page custom images:

1) Never modify built-in images or their names. Any changes made will be lost on the next Indigo Update.
2 )To create new device On/Off images, duplicate a built-in images and make their names unique in the indigoWebServer directory, or
3) Import new transparent png images into the indigoWebServer directory
a) Name the base image (that will be shown when the associated device is "off") something like "NewIconImage+.png"
b) If you want nothing to show when the device is "off", use a blank transparent png image for the base image.
c) Name the image to use for when the device is "on" to "NewIconImage+on.png"
4) After any changes are made to the indigoWebServer directory you must restart the Indigo Server and Client to see the new images in the image list.
5) Finally for the new or changed images to show up in the Indigo Touch app on your iPhone/iPad etc. open settings in the Touch app and select "Clear Cache"

Oh,

And if you use Photoshop to create or modify the images, a 2x Indigo Control Page Icon is approximately 0.15" wide
