i7 Submit to Indigo Domotics bug

i7 Submit to Indigo Domotics bug

Selecting the button to Submit to Indigo Domotics results in a webpage appearing with just the following text:

And a submit button but no fields to fill out. Clicking submit results in an error:

which is hidden behind the Indigo Domotics logo at the top left of the page. No other information was visible.
